Propranolol Hydrochloride is also indicated to treat or prevent heart attack, myocardial infarction, esophageal variceal bleeding and ascites, to reduce the frequency and severity of migraine headaches, cluster headaches, and for other medical purposes. It works by affecting the heart and circulation (blood flow through arteries and veins).

    Indications and usage:

    Propranolol Hydrochloride (Inderal, Inderal-LA, Innopran-XL) tablets and capsules are indicated for the treatment of:

  • Hypertension
  • Angina pectoris due to coronary atherosclerosis
  • Atrial fibrillation
  • Myocardial infarction
  • Migraine
  • Essential tremor
  • Hypertrophic subaortic stenosis
  • Pheochromocytoma


    Dosage and administration:

    Administer once (for long acting forms) or a few times daily at bedtime consistently either on an empty stomach or with food.

  • Inderal (Propranolol Hydrochloride) tablets 10 mg, 20 mg, 40 mg, 60 mg, 80 mg.
  • Inderal-LA (Propranolol Hydrochloride) capsules 60 mg, 80 mg, 120 mg, 160 mg.
  • Innopran-XL (Propranolol Hydrochloride) capsules 80 mg, 120 mg.


    Overdosage:

    Propranolol is not significantly dialyzable. In the event of overdosage or exaggerated response, the following measures should be employed:

    General: If ingestion is or may have been recent, evacuate gastric contents, taking care to prevent pulmonary aspiration.

    Supportive therapy: hypotension and bradycardia have been reported following propranolol overdose and should be treated appropriately. Glucagon can exert potent inotropic and chronotropic effects and may be particularly useful for the treatment of hypotension or depressed myocardial function after a propranolol overdose. Glucagon should be administered as 50150 mcg/kg intravenously followed by continuous drip of 15 mg/hour for positive chronotropic effect. Isoproterenol, dopamine or phosphodiesterase inhibitors may also be useful. Epinephrine, however, may provoke uncontrolled hypertension. Bradycardia can be treated with atropine or isoproterenol. Serious bradycardia may require temporary cardiac pacing.

    The electrocardiogram, pulse, blood pressure, neurobehavioral status and intake and output balance must be monitored. Isoproterenol and aminophylline may be used for bronchospasm.


    Contraindications:

    Propranolol HCl is contraindicated in:

  • cardiogenic shock
  • sinus bradycardia and greater than first degree block
  • bronchial asthma
  • in patients with known hypersensitivity to propranolol hydrochloride


    Warnings and precautions:

  • May worsen congestive heart failure
  • Avoid discontinuing therapy prior to major surgery
  • Diabetes: may mask symptoms of hypoglycemia
  • Bradycardia


    Adverse reactions, side effects:

    The most commonly reported adverse reactions of Propranolol ( >= 3% and greater than placebo) included the following: fatigue, dizziness, and constipation.

    Drug interactions:

  • CYP2D6, 1A2, or 2C19 enzyme inhibitors increase propranolol levels
  • CYP1A2 and 2C19 inducers decrease propranolol levels
  • Monitor prothrombin time when propranolol is co-administered with warfarin


    Use in specific populations:

    Pregnancy, labor and delivery: neonates whose mothers received propranolol at parturition have exhibited bradycardia, hypoglycemia, and/or respiratory depression.


    Patient counseling information:

    Advise patients:
  • Do not interrupt or discontinue using Inderal, Inderal-LA, Innopran-XL (Propranolol Hydrochloride) pills without a physician's advice.
  • Patients with heart failure should consult their physician if they experience signs or symptoms of worsening heart failure such as weight gain or increasing shortness of breath.
  • Inform diabetic patients that certain signs of hypoglycemia may be masked. Patients should report any changes in blood sugar levels to their physician.
